State the account, the amount, the dates and exactly what you want done. Attach the documents you have already supplied. Most disputes that get resolved are resolved at this stage, simply because a written complaint is harder to leave unanswered.
Escalate to the Licensing Body
If the operator does not resolve it, take the complaint to the regulator named on the licence you verified earlier. This is the practical reason for checking the register before depositing rather than afterwards.
Send the timeline with dates, the correspondence and the terms as they stood when you registered. A complaint that arrives assembled is treated very differently from one that arrives as a grievance.
Where It Realistically Ends
Beyond the regulator there is little practical remedy. Pursuing a foreign company through the courts over a few hundred pounds costs more than it recovers, and that is the honest end of the road.
Which is why the protective habits matter more than the remedies: verify early, withdraw regularly, keep your own records, and never leave a balance in an account larger than you would be willing to write off.
